Errors and recovery
Separate loading errors, streaming failures, and intervention flows.

Session and history-load failures belong to their request state. Failures that occur during an act belong to the timeline entry. Human intervention—tool approval, input collection, or agent authorization—uses the HITL contract rather than a generic error surface.
This distinction lets an integration show the right action at the right scope: retry a list request, show an act failure inline, or render an approval panel.
